I'm a huge DD fan, though admittedly not very good at it. I think giving good hero a minor temporary boon on a nat 20 wouldn't break the system at all, though I'm only really familiar with 2nd ed rules for Ravenloft, and never played 4th or 5th ed. I usually try and run my games somewhere in between...

The 3.5/Pathfinder rules for lycanthropes are supposedly balanced for PCs IF you follow the Level Adjustment rules. Sounds like a fun concept, especially for characters who kick butt for the lord.
Some seem pretty irredeemable however. Especially the Dreamlords. It'd definitely be a fun twist to throw at players who were familiar with the setting, to make them look at familiar enemies in a new light.
