Another doomsday device I've used, and that one in Ravenloft:
A kind of device that when properly fueled and set to work calls down a rain of fireballs and lighting over a large area.
That device was IMC built by a Marilith to trash a domain. She had worked over it for years.
She wasn't that dedicated, it was kind of a "hobby project" for her. She was doing whatever she wanted and gathering pieces and allies here and there for her project.
To fuel the machine of course there were souls + tears of innocents + other such things, so Zevarzia had a small cadre of cults under her to gather the "fuel", all working independant of each other (She wasn't paying much attention) others were nihilists, other worked under false promises and others were just intimidated.
What happened? The PCs failed to stop either the machine from being complete, the "fuel" from being delivered or the activation ritual. Well, the last part would be a complete suicide since Zevarzia would be there performing it and they were 10th lvl while she was CR 17-18. Anyway, Zevarzia activated the machine as the PCs (knowing it would be too late since it came to Zevarzia performing the ritual) found a vistani or something to take them out of the domain as the final hour was growing close.
A rain of fireballs ensued for 1 hour, with fireballs (20'radius, 6d6 dam ref DC 15) coming down from the suddenly bloodred clouds lighting strokes targeting random targets, people, trees, buildings etc (as if call lighting). The rain of devastation covered 2/3 of the domain or so trashing it completely. IIRC the "tiny cataclysm" took place over a 3000-5000 square kilometers area, killing about 30-40 thousand people (4/5 of the domain's population). Zevarzia has placed it in the wrong place, too close to the borders to cover the whole domain.
