The article begins with an in-character introductory foreword written by Freidrick Louis, a colleague of Dr. Illhousen. After that, each "chapter" begins with in-character Doctor Illhousen's notes on a particular patient followed by a gray-boxed DM text describing the character from an out-of-game perspective and includes stats for each.
