I know he has countless forms, but I only know these couple:
Tesla (Black Pharaoh?)
Bloody-Tongue
Haunter of the Dark
Maybe someone could tell me what the others are?
I remember reading (somewhere on Wikipedia) a table of his forms, but the page seems to be non-existant. Much thanks to anyone that helps.

If I remember right, I think Black just showed up in one adventure ("Look to the Future," in Shadows of Yog-Sothoth) and was only described as an occultist. While The Dark Tower gave Flagg's origin, I prefer to think of him as Nyarlathotep to the Crimson King's Azathoth. I prefer the old demonic Flagg from The Stand. I believe you mean "The Black Man of the Woods" - an avatar that is mentioned in Case of Charle Dexter Ward and appears in Dreams in the Witchhouse.
Shub-Niggurath is "The Black Goat of the Woods with a Thousand Young".

The Black Man of the Woods is correct, I was confused
