According to Strahd von Zarovich, Old Bones was a water elementa conjured and assigned the task of gathering bones in the sewers beneath Il Aluk. It performed this task for a hundred years before the Requiem occurred transforming Il Aluk into Necropolis. The energy unleashed killed the water elemental and reanimated it as a necromental. Furthermore, the wave of energy liberated the creature. Old Bones subsequently fled Necropolis and Darkon and made the Musarde River system, however avoiding the Vuchar River. In Barovia, Strahd has called for its demise.
Old Bones loves the rush of flowing rapids and strong rivers. It also enjoys collecting bones of those it kills, and it wears these like a human might wear a cloak. Bones rattling accompanying a roar, or the screams of its victims, denote that Old Bones comes near.[1]
