Introduction
First presented in the module Thoughts of Darkness, the illithid vampire is a fictional creature for the Ravenloft campaign setting for Dungeons and Dragons. These hybrids of illithids and vampires reside primarily within Bluetspur where they were born of biomancy. The High Master Illithid and Lyssa Von Zarovich are credited with the creation of this strain as part of a conspiracy to overthrow both the Illithid God-Brain and eventually Count Strahd von Zarovich. However, madness overtakes each illithid vampire, robbing it of most of its mental faculties and rendering it uncontrollable. Slain and discarded into the water dump of Bluetspur, the vampires revived and began to wander Bluetspur's surface of their own accord.[1] To make a bad situation worse, their numbers are constantly growing.[2]
Powers
Though their madness prevents illithid vampires from utilizing any sort of charm gaze or other subtle mind control, Illithid vampires hold some special abilities of both vampire and mind flayer. Like a vampire, their touch inflicts energy drain. They regenerate hit point damage every round. They have exceptional Strength, which increases with age like other vampires.[3] (However, given their recent creation, this is not really a factor of importance.)
Similarly to its living counterpart, the mind blast of an illithid vampire stuns its targets. However, it also requires the target to make a Madness Save.[3]

The tableau was of a beautiful young woman, scantily clad, beset by two unclothed, cuttlefish-headed humanoids. The one had torn open her neck and was about to lap with its veined, purple tongue at the blood that flowed freely. The other had wrenched open her skull and with its tentacles was ripping her brain from its seat. This was a death unlike any I had witnessed. And if I dared reach out and touch the orb, I too could savour the experience.

Lords of Madness (as Mind Flayer, Vampiric)
Characters with ranks in Knowledge (psionics) can learn more about vampiric illithids. When a character makes a successful skill check, the following lore is revealed, including the information from lower DCs.
DC Result 19 This colorless illithid is actually a feral predator called a illithid vampire. They can be distinguished from true illithid by their gray coloration and several physical mutations: long fangs circling their lamprey-like mouths, and the tip of each tentacle ends in a tiny, leech-like sucker. This result reveals all undead traits.
24 Vampiric mind flayers feed on brains, blood, and life energy — whatever presents itself. Like living illithids, vampiric mind flayers can generate a cone of psionic energy to stun their foes. A vampiric mind flayer drains blood by attaching its tentacles to a living victim, and its physical attacks drain life energy.
29 A vampiric mind flayer’s facial tentacles can grapple with an opponent and extract the brain in mere moments. The creature’s slimy flesh is unnaturally tough, but magic and silver weapons can pierce its defenses. A vampiric mind flayer’s wounds heal very quickly.
34 Vampiric mind flayers are resistant to cold and electricity. They are strongly resistant to magic. The origin of vampiric mind flayers remains a mystery, as is the question of whether they are capable of reproducing.
